Use bigger caps! Really they will vary by manufacturer.

Could be the nipple also. Had this problem on a Lyman and when I examined the nipple it had tiny ridges and grooves from the machining going around it. Sanded with finer grits until ridges were gone and cap comes off easier but still stays on nice.

Others may have other ideas.

If the nipple cone is smooth, check the clearance of the hammer nose around the nipple to be sure that the cap has room to fragment and come off the nipple. Many hammer noses also have a small "V" cut into their face to allow the nipple to release the cap.
