Ok question,

loading a Colt 44 cap and ball. Powder first, over powder wad, roundball, then grease over the roundball.

the question is, with the lubed powder wad is grease over the roundball to much? If no wad, grease over roundball, if you have a lubed wad, grease not needed.

Safety yes, do both as insurance. But are both recommended to ensure no chain fire?

Either wad or grease, both arn't needed. It is debated whether chain fires occur from the front. Loose or missing caps are probably the more likely culprit.
