when i had my chain fires it was the whole darn cylinder . A direct result of over loaded and not greased. The same load greased did not chain fire.
 
Wow! That would have gotten my attention for sure. Did the bottom ball make it out or just jam into the loading punch?
 
I suspect i did not have the loading lever installed. lead smeared all over the frame and side of BBL
